San Antonio behavioral health center cited for patient safety violations

San Antonio-based Laurel Ridge Treatment Center, a behavioral health and substance use disorder provider, was cited this summer by Texas health authorities after investigators identified several violations deemed an “immediate danger to patient health and safety,” according to a 50-page inspection report obtained by KSAT 12 News.

The report detailed multiple compliance issues, including staffing shortages that led to patient intake delays of four to six hours. The facility, which treats adults and children in mental health crises, was cited after investigators found patients had access to unsecure items such as paper clips, cords, a cigarette lighter, chemical sprays and — on one occasion — razor blades. 

Laurel Ridge’s current CEO, Ashley Sacriste, said in a written statement that the facility had implemented a corrective action plan and addressed all the issues. A follow-up visit in late September confirmed most concerns had been resolved, according to the report.
