The South Carolina Department of Public Health issued a citation-by-mail July 6 to Columbia, S.C.-based Recovery Solutions after its facility failed to submit a required plan of correction stemming from an investigation into a patient suicide, according to a July 8 DPH statement.
Recovery Solutions is a provider of inpatient behavioral health, residential treatment and community based services. It currently operates across 10 states.
Here are three things to know:
- DPH investigated a complaint and issued a report of visit May 8 that cited the provider’s Columbia Regional Care Center, a 347-bed forensic behavioral health facility, for multiple deficiencies after a patient died by suicide in November 2025. The report said the facility failed to submit its investigation of the patient’s suicide to DPH within seven days and did not have the incident report readily available during the department’s April 24 investigation.
- The department required the facility to submit a plan of correction by May 11. The department said it had not received the plan and issued a citation-by-mail July 6 for violating a regulation requiring facilities found noncompliant to submit an acceptable plan of correction. As of July 8, DPH said it still had not received the plan of correction.
- DPH said it does not confirm whether a facility is under investigation while reviews are ongoing to protect the integrity of its investigative process. The agency also said it has authority to deny, suspend or revoke licenses, assess monetary penalties or both for regulatory violations, but has not issued any enforcement orders against the facility.
