New York City-based NYC Health + Hospitals will expand its Bridge to Home behavioral health housing program to Brooklyn, adding a second site for unhoused New York residents diagnosed with serious mental illness.
The Brooklyn location will serve up to 50 guests with 24/7 on-site services, including stays of up to 12 months, according to a May 27 system news release shared with Becker’s. NYC Health + Hospitals/Woodhull providers will deliver behavioral healthcare, medical treatment, social services and housing navigation support at the site, which is expected to welcome its first guests in early fall.
In September, the first Bridge to Home site opened in Manhattan. Within the first seven months, more than 87% of current guests attended weekly clinical visits, approximately two-thirds completed housing applications, four guests were matched with permanent housing and three guests transitioned into permanent supportive housing, the release said.
The expansion will allow NYC Health + Hospitals to support up to 100 New York residents through the program, according to the release. Guests must be behavioral health patients within the health system, over the age of 18, diagnosed with a serious mental illness, experienced chronic housing instability and referred by a provider within NYC Health + Hospitals.
