Phoenix-based Southwest Behavioral and Health Services reported a hacking and IT incident involving email systems that affected 2,316 individuals. Notification of the breach was submitted May 20, according to HHS’ breach portal.
A forensic investigation determined that six employee email accounts were compromised, with the review of the affected email accounts completed April 30, 2026, The HIPPA Journal reported June 23. Southwest Behavioral and Health Services has since sent notification letters to the affected individuals and published a substitute breach notice on its website.
The organization did not disclose the types of information exposed in the incident. It said no evidence has been identified to suggest misuse of the affected data, according to the report.
