States with the highest out-of-pocket therapy costs

The average out-of-pocket cost for 60 minutes of psychotherapy was $174 in 2021, according to an analysis from Milliman.

The report, commissioned by mental health nonprofit Inseparable, compared the out-of-pocket costs for psychotherapy for self-pay, commercially insured and Medicare patients. 

Nationally, psychiatrists are less likely to accept insurance coverage of all types than other providers, Milliman noted in the report.
