American Dental Association adds mental health, GLP-1 prompts to patient forms

The American Dental Association has updated patient health history forms to include a mental health screening question and a prompt about GLP-1 receptor agonists.

The ADA Council on Dental Practice approved the changes in January, according to a March 24 news release from the association. The forms, last updated in 2021, are now available for purchase through the ADA store. 

One question asks whether patients have “felt connected to the world around them over the past two weeks.” A separate question asks whether patients have taken a GLP-1 receptor agonist. Responses may affect clinical decision-making and broader patient care planning.

To support the changes, ADA developed resources to help educate members on the topics. 

The organization recommends patients complete updated health history forms every two years with interim updates as needed.
