New York City-based NYC Health + Hospitals has opened two Elevate You clinics to provide behavioral and primary care services for people ages 16 to 25 as they transition between care levels.
The clinics, located at NYC Health + Hospitals/Woodhull and NYC Health + Hospitals/Queens, are backed by $4 million from MetroPlusHealth, according to a news release shared with Becker’s. The initiative is designed to keep youth engaged in care as they transition out of pediatrics, reduce emergency room visits and improve long-term health outcomes. Nearly 90% of youth stop seeing their mental health provider after turning 21, according to MetroPlusHealth claims data.
The Elevate You model offers behavioral health, reproductive care, vocational support and social services delivered by a multidisciplinary, consistent care team that includes a psychiatrist, nurse, social worker, youth peer and family advocate.
NYC Health + Hospitals provides inpatient and outpatient behavioral health services to about 8,000 children and adolescents annually and delivers more than 60% of the city’s behavioral healthcare, according to the release.
