Nashville, Tenn.-based Centerstone has expanded its Assertive Community Treatment program to multiple counties in Illinois to support adults with serious mental illness, according to an Oct. 29 report from River Bender.
The program now serves Madison, St. Clair, Jersey, Jackson, Union and parts of Franklin and Williamson counties. It is designed for individuals 18 and older whose mental health needs are not met by traditional services, including those diagnosed with schizophrenia, bipolar disorder or schizoaffective disorder. The program serves up to 60 adults with a focus on reducing psychiatric hospitalization and promoting recovery, according to the report.
Centerstone’s ACT team provides community-based services including crisis intervention, clinical treatment, medication management, peer recovery coaching, and housing and entitlement assistance. Fewer than 5% of participants “graduate” annually, reflecting its long-term therapeutic model, according to the report.
