Rochester, N.Y.-based University of Rochester Medical Center and Rochester Regional Health have partnered with Hillside, a Rochester-based provider of community-based services, to launch a care model for youths with complex needs transitioning from hospital to home.
The Next Steps Critical Time Transition Program, based at Hillside’s Monroe Campus, supports youths ages 11-17 through care-management services, transitional residential beds and connections to community care. Hillside is the first agency in New York to admit youths into the initiative, according to a Jan. 21 news release from Hillside.
Funded by the New York State Office of Mental Health, the program includes a community-based critical time intervention team and a transitional residential setting offering 24/7 support for eligible youths for up to 120 days. Community-based services have begun; the residential component will open soon.
The program plans to launch the model across a second location in Erie County, according to the release.
