Concert Health pulls data on a practice’s patients from referrals, prescriptions and screening to identify patients who could benefit from behavioral health services, according to a June 19 news release. Patients are then connected with a behavioral health manager through a phone or video call.

Through this partnership, 30 providers across Wisconsin’s Manitowoc and Sheboygan counties will use Concert’s behavioral health services.

At the Becker's Fall Behavioral Health Summit, taking place November 4–5 in Chicago, behavioral health leaders and executives will explore strategies for expanding access to care, integrating services, addressing workforce challenges and leveraging innovation to improve outcomes across the behavioral health continuum.
