The Virginia Department of Behavioral Health and Developmental Services revoked Richmond-based Hallmark Youthcare’s license Aug. 17, citing at least 27 violations, according to an Aug. 19 report from ABC affiliate WRIC.
The violations included several cases of neglect, two of which were tied to residents escaping the Goochland County psychiatric facility. On July 5, staff’s failure to intervene allowed a resident to obtain a staff member’s keys and leave the facility with seven other residents. On July 6, six residents exited the facility by forcing open a door with a chair after staff failed to intervene, according to an eight-page revocation letter sent to 8News.
Other violations included two instances of sexual activity between residents after failures in supervision, a staff member’s communication with a former resident in violation of Hallmark Youthcare’s professional boundary policy and health and safety deficiencies.
The department said Hallmark Youthcare also failed to inspect and repair broken doors and locks as required under an agreement that would have allowed the facility to operate under third-party oversight.
Within 10 days of Aug. 17, Hallmark Youthcare has the opportunity to dispute the revocation.
